* Your
child will receive the same care as my own children.
* I will
be available to all my patients either in my office or
via phone 24 hours per day 7 Days Per week.

Everyone will receive the same courtesy and respect from
me and my staff.
* Our
office will not have any hidden or deceptive costs.
*
If your child arrives before or at their scheduled
appointment time, he/she will not wait more than 10
minutes of their schedule appointment time.
* Any
requests and calls will be resolved within 24 hours.
FULL ORTHO TREATMENT
PROVIDED BY DR. T.J. EBERHARDT INCLUDES ALL OF THE
FOLLOWING SERVICES
* Ortho
checks every 6-12 weeks until treatment completed
*
Replacement of any loose brackets or broken wires
*
Emergency Ortho appointments.
*
Retainers with whitening kit.
*
Retainer evaluation every 6-12 months
* Any
necessary appliances.
* Ortho
re-treatment is no charge if retainers are worn as
instructed by Dr. Eberhardt.
* If
retainer(s) break due to lab error or wear
replacements
will be at no charge
* On time
finish with great compliance. |
